Clause RODO
In accordance with Articles 13 and 14 of Regulation (EU) 2016/679 of the European Parliament and of the Council of April 27, 2016. (RODO) we inform you that:

- Provision of medical services,
- Conducting patient registration,
- Customer Contact,
- Implementation of marketing activities.
Legal basis for processing:
- Customer/User Consent,
- Contract execution,
- Fulfillment of legal obligation,
- The legitimate interest of the Administrator.
Data recipients: Personal data may be transferred to entities cooperating with the Administrator to the extent necessary for the performance of services, including IT service providers and public authorities in case of legal obligation.
Data retention period: Personal data will be kept for the period necessary to fulfill the purposes for which they were collected, and thereafter for the period required by law.
Customer/User Rights: Each Customer/User has the right to:
- Access to their data,
- Correct their data,
- Delete their data,
- Restrict their data processing
- Data portability,
- Objection to processing their data,
- Withdrawal of consent to data processing.
The right to complain: The Customer/User has the right to lodge a complaint with the President of the Office for Personal Data Protection if he/she considers that the processing of personal data violates the provisions of the RODO.
Information about the requirement to provide data: Provision of personal data is voluntary, however, it is necessary to perform the services provided by the Administrator.
